We have been contacted by members within Health PEI that they may not have received Health Negotiation Proposal forms due to suspected delivery issues. As such we are extending the deadline until January 27th at 4:30pm to have your forms returned to the UPSE office at 4 Enman Crescent Charlottetown. You can find a printable copy of the form here as well as an online form option for you to submit directly.
